How to Register for the Running Team at Service

Go to school office to get your paperwork filled out (or download forms below) and turn signed forms into the school Activities Office. ALL necessary paperwork needs to be turned in BEFORE you are able to attend practice and/or get practice credit

REQUIRED BEFORE YOU START PRACTICE:

1. Physical Exam Paperwork
2. Sport/Athletic Participation Form
3. $175 Activities Fee paid to Service Activities Office
4. Submit Team Contract
5. Submit $35 team donation fee (give to coaches) More
